Community Infrastructure Questionnaire

Camden Water & Local Environment Questionnaire

Initial community fact-finding questionnaire about water, flooding, local infrastructure and residents’ observations.

Why we are asking

Local residents have raised concerns about water, flooding, local infrastructure and possible changes in health. We are beginning by finding out what people have actually seen, experienced or been told.

This questionnaire does not assume that these things are connected. We want to establish what is happening, where and when, before drawing conclusions.

Privacy: Please do not give your full address, names of other people, or private medical information. This questionnaire is designed for initial fact-finding.

Street or general area only — please do not give your full address.
